If you are interested in our services, call our office and speak to one of our office staff. They will ask you a series of questions to make the best match and accommodate your preferred schedule as we are able. Prior to your first appointment, you will need to complete intake paperwork. We can email that to you, mail it to you or you can download it and print it out by clicking on the links below. If you are being seen as a couple, please print out a copy for each individual. If the client is under age 18, please print out the Minor Intake packet.

FIRST APPOINTMENT

Please arrive five minutes prior to your first appointment so we can assure we have all necessary paperwork and make copies of your ID and insurance card. The first session will last approximately 75 minutes. Subsequent sessions are 45-60 minutes.

CANCELLATIONS

If you are unable to make a scheduled appointment PLEASE CALL US as soon as possible. We typically have clients on a Wait/ Cancellation List. We reserve the right to charge a $50 NO SHOW fee if appointments are not cancelled. This is not billable to your insurance and must be paid prior to your next visit.
