If you are interested in our services, call our office and speak to one of our office staff. They will ask you a series of questions to make the best match and accommodate your preferred schedule as we are able. Prior to your first appointment, you will need to complete intake paperwork. These forms are competed through the client portal. Please contact our office to set up your patient portal account. You can also request paperwork to be mailed to you as well.
Please arrive five minutes prior to your first appointment so we can assure we have all necessary paperwork and make copies of your ID and insurance card. The first session will last approximately 75 minutes. Subsequent sessions are 45-60 minutes.
If you are unable to make a scheduled appointment PLEASE CALL US as soon as possible. We typically have clients on a Wait/ Cancellation List. We reserve the right to charge a $50 NO SHOW fee if appointments are not cancelled. This is not billable to your insurance and must be paid prior to your next visit.
